Job Duties and Responsibilities:

  • Remotely monitor patients’ vital signs, symptoms, and health status daily using our dynamic portal.
  • Identify individual patient’s health trends by assessing vital sign data, symptoms, medication adherence, and other relevant information.
  • Engage in regular communication with patients to assess their well-being and address concerns. Escalate issues to patient’s doctor when necessary and/or according to established protocols.
  • Educate patients on their remote monitoring program, health devices, and health promotion.
  • Maintain accurate and detailed notes on patient charts in our portal and the electronic health records (EHR) system.
  • Collaborate with healthcare professionals, including physicians and nurses to ensure seamless coordination of care for patients.
  • Provide technical support and guidance to patients regarding remote monitoring devices, ensuring they understand how to use the equipment properly.
  • Adhere to all applicable healthcare regulations and patient privacy guidelines, ensuring patient information remains confidential and secure.
  • Participate in continuous quality improvement initiatives, suggesting enhancements to remote patient monitoring processes and protocols.

Skills and Qualifications:

  • US Registered Nurse credentials required.
  • 3 years Prior experience with remote patient monitoring technologies and BPO telemedicine platforms is advantageous.
  • Comfortable using remote monitoring devices, video conferencing software, and electronic health record systems. Ability to troubleshoot basic technical issues is a plus.
  • Excellent verbal and written communication skills. Meticulous attention to detail, ensuring accurate collection and documentation of patient data.
  • Demonstrated ability to provide emotional support to patients and families, showing empathy and compassion in all interactions.
  • Strong organizational and time management abilities, with the capability to handle multiple tasks efficiently.
  • Maintain a high level of professionalism, integrity, and ethical conduct in all aspects of the job.
